Photo Release Form
I hereby grant to the Children’s Museum of South Dakota and to the educational staff the right to photograph my dependent(s) and/or use the photographs and stories in its educational materials, promotional materials, and publicity efforts. I understand and agree that these materials will become the property of the Children’s Museum of South Dakota and will not be given to me. I understand that the photograph(s) may be used in a publication, print ad, direct-mail piece, electronic media (e.g. video, CD-ROM, social media), website media, or other form of promotion. I understand that my dependent’s last name or identifying information will not be connected to any of the forms of media when used for education or promotional materials.
I hereby authorize the Children’s Museum of South Dakota to edit, copy, exhibit, or publish the photos for lawful educational and promotional purposes. I waive the right to inspect or approve the finished project, including written or electronic copy, wherein my dependent’s likeness appears. Additionally, I waive any rights to compensation arising or related to the use of the photos for educational or promotional purposes.
I hereby hold harmless and release the Children’s Museum of South Dakota, the photographer, their offices, employees, agents, or designees from liability for any violation of any personal or proprietary right I may have in connection with such use.
I hereby certify that I am the parent or guardian of the child named above and do hereby give my consent without reservation to the Children’s Museum of South Dakota on behalf of this person.
